When our patients make an appointment and prior to your appointment our staff will ask a brief questionnaire to determine if your appointment can continue as scheduled. We encourage our patients to be prepared prior to their appointment. Our staff will recommend going online to www.beyereye.com to print and complete the appropriate forms prior to your routine/comprehensive eye exam. Please have your updated medical insurance card and vision plan information/card (if applicable) ready prior to your exam. If you do not have a vision plan please call your medical insurance to determine if you have routine coverage with your medical insurance (ask you insurance if you have coverage for CPT Code Z01.00). Knowing this information is imperative for proper billing and expediting your exam in our office.

When you arrive for your appointment please call our office from your car and our staff will inform you when to enter the office. Please come inside alone unless the exam is for a minor (under age 18) and/or need someone to aid in mobility and/or communication. Every patient will have their temperature taken and must wear an approved face covering. Each patient will complete a brief questionnaire about their health in the office. We offer an elective digital retinal exam (no drops) to reduce exposure time and promote social distancing.

Beyer Eye Associates will be disinfecting exam rooms and equipment between patients and cleaning high contact areas frequently throughout the day.
